Causes for network problems?

Server owners such as Falcon Online had great problems in campaign, caused by slow client connections. As we know, Falcon BMS has a p2p architecture where clients exchange data among each other.
What was/is the cause of breakage they’ve encountered? In what way does the software’s internal logic cause a failure?
This reminds me of a long-standing issue with my own software: adding a flaky connection made it send slower. There was method in that madness.
