I do one thing… I always unplug my hotas, MFDs cougar from the USB, and when plug again whn I go to run falcon, I plug in the same order and the same usb, and since I do that thing… that never happen to me again

Yup, this is a problem I’ve suffered as well. It’s Windows re-ordering your devices based on USB ports, probably the alignment of the stars, etc. As Revientor says, plugging in devices in the same order and ports can help.
You can also try this program, JoyID:
http://theairtacticalassaultgroup.com/forum/showthread.php?t=13009It allows you to specify the device order.

The following procedures should make managing the gaming peripherals much easier:
-
Plug all of the controllers and other game related peripherals (e.g. TrackIR) into a single POWERED multiport USB hub such as this one:
http://www.amazon.com/AmazonBasics-Port-USB-power-adapter/dp/B00DQFGJR4/ref=sr_1_4?ie=UTF8&qid=1445547257&sr=8-4&keywords=powered+usb
Then plug the powered USB hub into one of the PC/laptop’s USB ports, always using the same USB port on the PC/laptop. -
Start any controller, TrackIR, etc software, load the profiles of choice as needed into the peripherals, then check calibration, button functionality, view tracking, etc.
-
When all A-OK with the controllers, TrackIR, etc, start F-BMS without shutting down any of the controller and TrackIR software.
Also recommend taking screen shots of all the settings when they are all to your liking.

Also, make sure your keyfile IS NOT set to “read only.” Had that happen during a recent install of falcon.

Unplugging stuff can mess the order up.
You need to keep ALL your controllers (for BMS and everything else) in the same order. Even if you unplug a gamepad and it messes the order of your BMS controllers, all your settings will go to default. I keep all my stuff plugged in and never change their USB location. I also backup my config folder. If it gets messed up from unplugging something, just plug the controller back in and replace the config folder with your backup. All will be well.

Here’s what you have to do:
1. Plug in all the peripherals you need, load up the necessary profiles (TIR, TARGET, etc.), and so on.
2. Start up Falcon BMS. Make sure your axis/buttons/controllers are assigned properly. Go into the game (3D world) and fly around for a bit and test stuff just to be sure they are assigned properly.
3. Exit BMS.
4. Go into your \Falcon BMS 4.32\User\Config directory and make axismapping.dat and joystick.cal read-only.
I have done this a long time ago and have never had a problem with BMS despite moving the computer to a different location 3x during the last 2 years. I’m not 100% sure it will work if you plug/unplug your stuff on a daily basis though but it should.
Good luck!
